The plan may involve better support, control of infection, a bone graft, or a mix of these steps. A bigger plate is not a diagnosis. Nor is another month of waiting a plan. After fracture surgery, the real question is why the bone has not joined and what must change. This guide explains the main choices for adults with a fracture that has failed to heal. It also sets out what to ask at a review. The goal is more than a better X-ray: less pain, a useful limb, and a safe return to daily life matter too. The right approach depends on the bone, the wound, and the person, so a surgeon must assess the case before choosing a procedure.

Is it delayed union or a fracture that will not heal?

Delayed union means healing is taking longer than expected. Nonunion means the fracture has failed to unite. Those labels are not set by the calendar alone. Different bones, injury patterns, and prior procedures heal at different rates. A surgeon compares repeat X-rays with pain, function, and the clinical exam. A visible fracture line on one image does not tell the whole story. Neither does pain by itself. Before nonunion treatment, bring earlier scans and the dates of each operation to the review. Ask whether new bone has formed over time and whether the implants still hold the pieces in place. A CT scan may help when plain films leave doubt. The key is a clear account of progress, or the lack of it, rather than a blanket claim that every fracture should heal within three months.

Why can bone healing fail after surgery?

Bone needs a sound local setting to heal. Poor blood flow, a gap, damaged tissue, or too much movement can block that process. An open injury may add both tissue loss and infection risk. Smoking, poor nutrition, and poorly controlled diabetes can also make the task harder. Yet blame is a poor tool. A nonunion can occur even after good care. Nonunion treatment should therefore test the likely causes rather than assume that the patient or the implant is at fault. More callus, the new bone around a break, may suggest a different mechanical problem from a site with little new bone. Appearance still does not rule out infection. Think in three parts: stability, blood supply, and infection status. A plan that tackles only one of them may leave the main obstacle untouched.

Figure 1. Fever or feeling very unwell adds urgency. Infection may also be less obvious, so a quiet skin surface is not proof that the deeper site is clear. Assessment can include blood tests, imaging, and tissue samples when surgery is needed. In an infected case, nonunion treatment must address both the germs and the unhealed bone. The AO Foundation guidance on fracture infection stresses removal of dead or infected tissue, suitable stability, and care of the wound. Antibiotics support that work; they do not replace it. Some cases need several stages. Others may suit a single planned procedure. The choice rests on the tissues, fixation, organism, and general health, not on a promise that one route is always faster.

When does revision fixation make sense?

If the bone ends move too much or sit in poor alignment, the support may need to change. Revision fixation can involve a new plate, a different nail, or an external frame. The choice depends on the site and the reason the first construct has not worked. A nail may suit some shaft problems. A plate can offer another way to restore alignment and support. A frame may help when access to damaged soft tissue or management of a bone defect is important. None is a universal winner. Effective nonunion treatment matches the method to the problem and protects the remaining blood supply. Ask what the next operation will change: contact, alignment, stability, or all three. If the answer is only “stronger metal,” ask for the missing part of the explanation. The implant supports healing; it cannot do the healing itself.

Figure 2. Nor can graft material make an unstable construct sound. Bone taken from the patient provides useful biological properties, yet harvest adds another surgical site and can cause pain. Donor bone and substitutes offer other options, with different roles and limits. In nonunion treatment, the important question is what the graft is meant to solve and how it fits with the rest of the plan. Size of the defect, tissue health, prior surgery, and infection all affect that choice. Do not confuse a product list with a treatment strategy. Ask why a given material was chosen, what tradeoffs it brings, and whether further procedures may be needed. More graft does not automatically mean better healing.

Figure 3. These devices use electrical, magnetic, or ultrasound signals, depending on the model. They are not a way to ignore loose fixation, a large unresolved gap, or an infected wound. The FDA classification for non-invasive bone growth stimulators describes prescription devices and their intended uses. Suitability still depends on the specific device and clinical case. In nonunion treatment, ask what result the team expects, how progress will be checked, and when the plan will be reviewed if healing does not advance. Be equally direct about injections or cell-based approaches. A promising idea is not proof of benefit for every fracture. Ask for evidence tied to the proposed use, including harms and uncertainty. Novelty alone is a weak reason to accept a costly procedure.

What should recovery and follow-up look like?

There is no honest recovery date that fits every unhealed fracture. The bone involved, extent of tissue damage, infection, and chosen procedure all affect the path ahead. Nonunion treatment should come with a clear plan for weight bearing, wound care, movement, and follow-up scans. Do not test an operated limb by adding load on your own. Ask which activities are safe now and which require evidence of healing first. Rehab should protect the repair while helping restore strength and useful motion. Good food, support to stop smoking, and control of relevant health problems belong in that plan too. Keep a simple recovrey log of pain, swelling, and function to discuss at visits. Seek prompt help for new drainage, spreading redness, fever, or a sharp change in pain. Progress must be checked, not assumed.

Questions to ask before choosing the next step

A useful consultation ends with a reasoned plan. It should explain what has failed, what will change, and how success will be judged. Nonunion treatment is not a contest between plates, nails, grafts, and frames. It is a choice about which tools address this fracture’s problems with acceptable risks. Ask the team to separate facts from uncertainty, including whether infection has been assessed and how much healthy bone remains. Discuss pain relief and function as well as union. If the proposed course is complex, a second opinion from a surgeon who treats difficult fracture healing may help clarify the options. Take notes. Bring someone who can help you remember the answers. The checklist below keeps the discussion focused on decisions that matter.
